Extended Care Unit
When the need for you to recover from illness, injury or surgery arrives, Pomerene’s Extended Care Service allows you to recover at your pace, close to home with easy access to nursing and therapy care. Our goal is to assist you and your caretaker in becoming as independent as possible before returning home.
Post-Acute Care Services
- Acute nursing care for major illness or surgery
- Access to short-term, intensive physical and occupational therapy prior to going home
- Support with IV infusions
- Assistance returning to independent living and activities of daily living
- Access to in-home physical and occupational therapy
Hospice Care
- Coordinate with Hospice providers for those who desire to use hospice care for comfort and symptom management
- Access to Hospice service providers. Please contact your Hospice provider if you need inpatient hospice symptom management.
- Hospice Respite Care – providing caregivers a temporary rest while their loved one is in a safe environment.
